RE: Amendment and Plan Addendum South Florida Community Care Network, LLC d/b/a Community Care Plan ("CCP") (Health Plan)

Dear TNFL Provider:

As a longstanding partner with Community Care Plan (CCP), Therapy Network of Florida (TNFL) is contacting you at this time to make you aware of some upcoming changes that will impact you as a participating provider in our network.

Effective January 1, 2026, all Behavior Analysis (BA) services for members enrolled in CCP will be managed directly by CCP. Providers currently participating with TNFL who wish to continue serving CCP members may contact CCP at CCP.Provider@ccpcares.org.

Please note: In the coming weeks you will receive additional information regarding the authorization process and claims submission guidelines that shall be in place after December 31, 2025.

In addition, as part of our combined efforts with CCP to ensure the sustainability of the provider network and to align with current market conditions, we are implementing an adjustment to the reimbursement rates for authorized covered services for the specialty of Behavior Analysis.

Effective September 1, 2025, authorized covered services for the specialty of Behavior Analysis will be reimbursed at 80% of the Medicaid Fee Schedule. No further action is needed from you at this time.
